“My name is {your full name} and today is {today’s date}. I consent to this video for use as legal evidence in the future. I promise to do the following:
- I will not solicit client for any work outside of the job that I am assigned.
- I agree to work with the client only through Wild Entertainment for a period of 24 months.
- I will not share any company confidential information with any third party.
- I have been advised not to consume any food or drinks from the client.
- If I do consume any food or beverage then I am 100% liable for any effects upon my health and actions.
- I will not solicit sex from the client or perform sexual acts in exchange for money or gifts.
- I will arrive to my gig sober and not under the influence of drugs or alcohol.
- I will be aware of the customer’s comfort level and will behave in a manner that is appropriate.
- I will not sexually harass the customer in any way, shape or form.
- REGARDING ADULT ENTERTAINMENT
- I will not expose myself to the customer unless they have requested and paid for a fully nude option.
- I will not send nude pictures of myself to the customer without first obtaining consent.
- When performing in the nude, I will not physically make contact with a customers mouth, breasts or genitalia with my mouth or hands or genitalia.
- I will not masturbate in front of the customer unless it is requested and paid for as a fantasy show.
- I am aware of the laws of the state that I work in regarding adult entertainment and I will abide by them.
